1. Visit your local SASSA office
2. Explain to them what you need.
3. They will give a request for bank payment form to be filled by your bank and instructions on what to do next.

EDIT: You can also download the FORM here > Print it out on an A4 paper > Take it to your bank(Capitec) > From your bank to your local SASSA office.
